Notice of Privacy Practices
This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.
What this notice is for
As a residential care provider we hold health information about the people who live here. Federal law requires us to protect it, to tell you how we use it, and to tell you what rights you have over it. This page is where that notice is published.
Your rights over your health information
Broadly, you have the right to see and get a copy of your records, to ask us to correct them, to ask for a list of certain disclosures we have made, to ask us to limit what we share or how we contact you, to receive a paper copy of this notice, and to complain — to us, or to the U.S. Department of Health and Human Services — without being penalised for it.
How to get the notice, or ask a question
A copy is available at the community and we will post the full notice here. To ask for one now, to exercise any of the rights above, or to raise a concern about how your information has been handled, call (714) 775-6775 and ask for the administrator, or write to Westbrook Park Senior Living, 15302 Brookhurst Street, Westminster, CA 92683.
The full notice is being prepared and must be supplied by Oxford Health Group. Do not publish this page as a finished notice.
This is the only page on the site that cannot be written from the outside. A Notice of Privacy Practices has contents prescribed by 45 CFR 164.520, it has to describe the uses and disclosures this community actually makes, it names the community's own privacy contact, and it carries an effective date and legal effect from the day it goes up. The summary above is orientation for a reader who lands here; it is not the notice.
Oxford will already have one for its other communities — that is the document to localise, not another operator's. It needs the entity name, the privacy contact, the complaint route and the effective date checked line by line for this community before it goes live.
When it lands: paste it in above, delete this panel, and add dateLabel="Effective" with the adopted date. The date is omitted right now on purpose — an effective date on a notice nobody has adopted is itself a false statement.
